Reasons for extracted permanent teeth in Korean population



Abstract
As a geriatric and a chronic disease are rising the main social problems in the medical security system, the control of these diseases becomes to be important issues of the modem society. In order to maintain the oral health for Korean, it is important to detect and remove the cause of teeth extraction. Dental health is considered to be one of the most important part as total health. Therefore it is needed in national level to find the cause of tooth extracted.
The purpose of this study was to analyze the cause of tooth extracted in Korean population. 2 community dental practioners per 4 each rural areas was chosen randomly, because there lives 78% of Korean. 4 each rural areas were Seoul, Inchun, Ansan, and Chonju having a population more than half a million, as the metropolitan and large cities. 1 community dental practioner per each 4 urban areas was chosen randomly, because there lives 22% of Korean. 4 urban areas were Kuri, Chungeub, Kimjae and Buyeou having a population less than 200 thousands such as complexed cities of city and rural community. In the complexed cities, half of the population lives in rural area and the other half in urban area: To ensure extracted teeth directly by visual inspection, the author collected 4,676 permanent teeth from 4,452 patients, extracted by general dental practitioners.
Among 4,676 extracted teeth, 4,654 teeth were made the object of this study except 22 teeth due to supemumery tooth. The author analyzed the cause of teeth extracted by age, sex, area, tooth-kind. The cause of teeth extracted were dental caries, periodontal disease, dental caries with periodontal disease, orthodontic treatment, trauma, preparation of prosthetic treatment and others. The results obtained were as follows:
1. Among the causes of tooth extraction, dental caries was 40.1%, periodontal diseases 22.2%, and dental
caries with periodontal diseases 4.8%.

2. Dental caries was the major cause of tooth extraction and continues throughout life for both sex, but
periodontal disease was remarkably increasing in those aged 35year old and over.
3. The mean number of tooth surface having active caries was higher in the extracted teeth due to dental
caries than due to periodontal disease.
4. Although the third molars were most extracted in the all kinds of teeth, the main reasons of the extraction were pericoronitis impaction and dental caries. Especially, the third molars were extracted, due to only the cause of third molars itself more than other reasons.
5. The number of root rests were 23% of total extracted teeth. More teeth were extracted in the male than female and on the age of 55-64 year than other age groups.

Because dental caries and the periodontal disease are the major cause of teeth extracted in Korea, the dental health policy should be focused on the prevention and treatment of these two diseases in the government level.
